1. Before completing the form below, review all information under PARENTS and CODE OF CONDUCT as you and your wrestler(s) will be required to sign off that you have read and agree to the outlined terms.

2. Carefully fill out the form below, and pay attention to birthdate year, to ensure correct age division (affects OKWA membership and tournament entry).

3. Complete one form per wrestler (do not combine siblings).

4. You will be directed to PAYMENT tab and there you may pay for siblings together to receive discounted membership.

**First year wrestlers are required to provide a copy of birth certificate at first practice, to be kept on file by the club.
